Rewards Program Terms and Conditions

1. Introduction

Welcome to our Rewards Program ("Program"). By opting into the Program, participants ("you") agree to be bound by these Terms and Conditions. The Program is designed to reward our loyal customers for their purchases of medical supplies from Medical Innovations ("Company", "we", "us").

2. Program Eligibility and Enrollment

2.1. The Program is open for enrollment to all customers, excluding wholesale customers, of the Company subject to the following conditions:

- The Program is opt-in. You must enroll to participate.

- Enrollment in the Program is subject to acceptance of these Terms and Conditions.

2.2. Enrollment begins on the day of enrollment and is ongoing. The rewards are not retroactive and will only apply to purchases made after joining the Program.

3. Earning Rewards

3.1. For every $500 spent on eligible purchases from the Company, you will earn a $20 gift card ("Reward").

3.2. Eligible purchases include all medical devices and pharmaceutical products offered by the Company, excluding taxes, shipping, and handling fees.

3.3. The options for the Reward include a Visa Prepaid Card, Amazon, Starbucks, Grubhub, DoorDash, and UberEats gift cards. Availability of options is subject to change.

4. Reward Redemption and Delivery

4.1. Rewards will be automatically calculated and issued within 24 hours of reaching the $500 spending threshold.

4.2. Rewards will be delivered electronically to the email address associated with your Program account.

4.3. The Rewards email recipient is managed by the customer's primary account holder.

5. General Conditions

5.1. Rewards are not transferable, exchangeable, or redeemable for cash.

5.2. It is your responsibility to ensure that participation in the Program does not violate your company's policies or any laws applicable to you. The Company will not be held liable for any breach of such policies or laws by you.

5.3. The Company reserves the right to modify, suspend, or terminate the Program at any time without notice, subject to applicable law. Changes to the Program will be communicated via email or through our website.

5.4. The Company is not responsible for any technical failures or problems that may affect your ability to earn or redeem Rewards.

6. Limitation of Liability

The Company shall not be liable for any loss or damage arising from your participation in the Program, to the fullest extent permitted by law.

7. Governing Law

These Terms and Conditions shall be governed by and construed in accordance with the laws of the state of Maryland, without giving effect to any choice or conflict of law provision or rule.
